- Mileage Reimbursement: $0.43 per mile
- Coverage Area: Clinton, Cumberland, and Monroe Counties, KY
- License Requirement: Active Kentucky Physical Therapist License or Compact State License

Position Summary
The Physical Therapist (PT) is responsible for providing skilled physical therapy services to patients in the home health setting. The PT evaluates patients, develops individualized treatment plans, delivers therapeutic interventions, and collaborates with physicians and interdisciplinary team members to help patients achieve their highest level of independence and functional mobility.
Key Responsibilities
- Perform comprehensive physical therapy evaluations and assessments based on physician orders and referrals.
- Review patient diagnoses, medical history, precautions, and contraindications to establish appropriate treatment plans.
- Develop measurable goals and individualized care plans designed to improve patient outcomes and functional independence.
- Deliver skilled physical therapy interventions to enhance mobility, strength, balance, endurance, and safety.
- Reassess patient progress regularly and modify treatment plans as necessary to achieve optimal outcomes.
- Communicate patient progress, treatment recommendations, and discharge planning with physicians, caregivers, and interdisciplinary team members.
- Recommend and assist with obtaining adaptive equipment and assistive devices to improve patient function and quality of life.
- Maintain accurate and timely clinical, treatment, and billing documentation in accordance with company policies and regulatory requirements.
- Ensure compliance with HIPAA, company policies, professional practice standards, and all applicable state and federal regulations.
- Supervise Physical Therapist Assistants (PTAs) and therapy aides in accordance with state practice guidelines.
- Participate in staff meetings, continuing education programs, and professional development activities to maintain clinical excellence.
